Tree of Hope is now hosting Divine Care Services as the Primary Care Provider for the adult population in our office. Services include:

Divine Care Services provides:

• Preventive Health

• Acute Care Management (Cold symptoms, Sinusitis, UTI's, Flu)

• Mental Health

• Chronic Care Maintenance (Diabetes, Hypertension, Asthma)

• Pre-op Physicals, Tuberculosis Screenings and COVID testing

• Diagnostic tests, labs and imaging review

• Dept of Transportation (DOT) Exams - Coming soon

Accepted Insurances:  Medicaid, Medicare, United Healthcare, Cigna, BCBS/CareFirst, and uninsured with self pay discounts. Book an appointment below.
